Admission Criteria

Greater Unity Adult Services is a Medicaid provider that allows individuals with ID/DD Waiver funding to enroll and participate in the program. Additionally, the Day Support Program will admit those who do not have Medicaid funding and those who require private pay situations designed to meet their specific needs.*

The staff at Greater Unity works closely and diligently with support managers and family members to ensure each client admitted to the program has their individual needs met from the outset. In order to be considered for admission, individuals must:

  • Be at least 18 years of age
  • Have a primary diagnosis of Intellectual Disability
  • Be dually diagnosed with ID/DD conditions
  • Be eligible for ID/DD Waiver funding or private pay

Each of our services are guided by an application and careful assessment process. Based on the individual meeting the minimum criteria for admission, the organization will carefully coordinate the program with all parties involved for a smooth intake and transition into the program.
